Bring your insurance card and photo ID
Auto Accidents & Personal Injury Cases: Were you injured in a car accident? Your chiropractic care may be covered through your auto insurance MedPay benefits or personal injury claim. We work directly with auto insurance carriers and attorneys to make the process simple for you.
Patient Forms
Once we gather your contact information, we’ll email you the new patient paperwork so you can complete it ahead of time and save time in the office.
If you’d prefer to fill it out in person, let us know and plan to arrive about 20 minutes early to your first appointment.
Location & Parking
We’re located on the first floor of One City Center in Monument Square, Downtown Portland — right next to the fountain.
Parking Options:
One City Center Garage – Validated for patients!
We provide 1 hour of free parking in the One City Center Parking Garage only.
Bring your parking ticket with you to your appointment for validation.
Note: The garage is detached from our office. It’s a short walk across Monument Square to our office building.
Street Parking – Metered spots are often available nearby.
Free between 6:00 PM and 9:00 AM.
FAQs
Do I need a referral? → Usually no. You can book directly. Exceptions: MaineCare and VA Community Care patients do need a referral.
Will my insurance cover chiropractic? → Often yes. We’ll verify your benefits and explain your coverage before starting care.
What should I wear? → Comfortable clothing you can move in — think workout or casual wear.
Do you take same-day appointments? → Yes, when available — call us and we’ll do our best to fit you in.
